Important Information for Patients and Caregivers

What you should know.

To Patients:
As a patient new to medical marijuana, we understand you have lots of questions - from what you need to do to qualify, to what kinds of products are available, to how they work. At Verilife, you can trust that we will answer all of your questions with the utmost truth and transparency. We’re here to help you get started on living your best life. To Caregivers:
As a patient caregiver, there’s a lot to know about administering medical marijuana and how it impacts your patients' well-being. To help those you care for live their best life, we have the information you need to answer any questions you may have. Be sure to always keep a log and share it with your patient’s physician.

You have the option of designating a personal caregiver to assist you with your medical use of marijuana.

How to qualify as a patient caregiver in Pennsylvania

To qualify for a designated caregiver registry identification card, a designated caregiver must:

  • Be at least 21 years of age
  • Minor patients (under the age of 18) are required to have a parent or legal representative serve as their caregiver
  • You can be a caregiver to up to five qualifying patients
  • The registration fee for caregivers is $50
There are four types of caregivers 
Adult patient caregiver registration

You are registering for an adult patient who is already in the system. You will need to enter the patient's name, date of birth and patient ID number when registering.

Caregiver registering for an existing patient 

You are registering for a patient who is already in the system but will not visit a dispensary. Patient will not register separately, nor will they receive an ID card in this instance. You will need to enter the patient's name, date of birth and patient ID number when registering.

Caregiver registering for a new patient 

You are registering for a patient who is not yet in the system and will not visit a dispensary. The patient will not register separately, nor will they receive an ID card in this instance. You will need to enter the patient's name, date of birth, address and telephone number when registering.

DOH-approved caregiver registration 

When you register under this category, you are agreeing to allow the Department of Health to share your contact information with patients in your area who need a caregiver. If this is not your intent, you should choose one of the three options above.
