How to Get a Maryland Medical Cannabis Certification

Do you Qualify for a medical cannabis certification?

  • Are you at least 18 years of age (or are you a minor with an adult caregiver and have a physician certification)?
  • Are you a legal resident of Maryland with proof of residency?
  • Are you registered  with the Maryland Medical Cannabis Commission (MMCC)?
  • Have you been diagnosed with one of the qualifying conditions?
  • Have you been certified by a Maryland physician who is registered with the Maryland Department of Health to recommend medical cannabis?

Register as a Patient with Maryland Medical Cannabis Commission (MMCC)

You can apply for your Maryland medical cannabis certification online through the Maryland Medical Cannabis Commission Registry. It's the fastest and most convenient way to register.

To register you will need the following:

  • Electronic copy of a valid US government issued photo ID and proof of Maryland address
  • Photo of yourself (photo must satisfy photo requirements) 
  • For hospice patients in lieu of a recent photo, Hospice patient may submit a letter from the Attending of Hospice Record (AOHR) provider on hospice letterhead
  • Last for digits of the patients Social Security Number

Go to the Maryland Medical Cannabis Commission Registry and click the 'Create Account' link under the 'Patients' heading.

  • Complete the 'Before You Begin' section to review and confirm that you have the required documents and information ready for upload.
  • Complete the online form, uploading requisite files.

You will receive an email after the MMCC has reviewed your application indicating whether or not your application has been approved. If you are approved, you can schedule a visit with a registered provider to obtain a written certification.

You can visit Verilife to purchase medical cannabis. 

The certification issued by a provider for a patient identifies the amount of dried flower and THC that the patient may purchase in a 30-day period. The limit is calculated as a ROLLING 30-day limit  not by calendar month.

Once you receive your approval you can log in to the MMCC Patient Registry to select a caregiver.

A patient may designate up to two persons as caregivers.

Caregivers must be 21 years or older.

After the provider has examined the patient and the patient’s records, the provider may issue the patient a certification immediately.​
