How to Get an Illinois Medical Cannabis Card

Do you qualify for a medical cannabis card?

  • Are you at least 18 years of age (or are you a minor with an adult caregiver and physician certification from two physicians)?
  • Are you a legal resident of Illinois with proof of residency?
  • Have you been diagnosed with one of the qualifying conditions?
  • Has a physician certify that you have a qualifying condition, and have you established a bona fide relationship with that physician?

Opioid Replacement 

In 2019, patients who would otherwise be prescribed an opioid can now choose to seek relief with medical cannabis and are eligible to apply for a medical cannabis card.

Frequently Asked Questions

If you already have a card, you only need to apply for a medical cannabis card extension. The Illinois Department of Public Health will mail you a renewal packet around the time of your expiration date. Simply return your renewal information a minimum of 45 days prior to the card’s expiration date. Your renewed card will be valid for two additional years. If approved, you will receive your medical cannabis card in the mail and can visit your designated medical cannabis dispensary.

Visit Verilife or the dispensary you selected during the application process. You must have your medical cannabis card and state-issued ID with you in order to purchase medicinal cannabis products.

Registered qualifying patients and their designated caregivers can obtain or possess 2.5 ounces of medical cannabis during any 14-calendar-day period. 

A patient may petition the Illinois Department of Public Health to increase the adequate supply by completing a waiver form with a check for $25.

The waiver must be completed by your physician.
